 1. 下列文件及報名費必須與申請表一併提交：

a) 出生/ 身份證明文件副本 

b) 兩張(1.5 x 2.0 吋)半身近照 

c) 報名費：港幣$100正 

 

2. 所有報讀的年幼學生於面試時，必須足齡，否則將不獲安排面試。

3. 填妥的表格可以: 親臨遞交 或 郵寄方式

如以郵寄方式申請，務必連同上述 (第1項) 文件及報名費一併提交，並請以劃線支票繳付，支票抬頭請寫 " Tiffany Lau Vocal Performance Academy "。

申請人的面試時間安排，將透過電話個別通知。

​

所有報讀任何班別之學生，經由學校審核後，學校將保留安排最合適入讀班別給申請者之權利，學生或家長不得異議。

​

所繳付之申請費用一概不予退還或轉讓。

 

Admission Procedures:

The application form can be obtained from the general office or downloaded from the TLVPA’s website.

The following documents / items have to be submitted together with the application form:

a) A copy of the birth certificate 

b) Two (1.5 x 2.0 inches) recent passport photos (Please paste one on application form)

c) Application fee: $100 
 

Applicant should meet the age requirement.

The completed form can be submitted in person or by post

If your application to be submitted by post, the completed documents along with a crossed cheque of $100 to “ Tiffany Lau Vocal Performance Academy’’.

The audition arrangement for applicant will be informed by phone.

Unless specified, TLVPA reserves the right of class arrangement.

The application fee is non-refundable.
